I understand that Little Sprouts Preschool is a small, in-home preschool program located in Mesa, Arizona, and that my child will take part in everyday preschool activities including indoor and outdoor play, art and sensory materials, movement and music, water play, and use of age-appropriate play equipment. I understand that these activities carry inherent risks, including the risk of minor injury, illness transmitted between children, or accidental damage to personal belongings, even when reasonable care and supervision are provided. In consideration of my child being accepted into the program, I voluntarily assume those ordinary risks and release Little Sprouts Preschool, its owner, and its staff from liability for injury, illness, or loss arising from ordinary participation in the program. This release does not apply to gross negligence or willful misconduct.

In the event of an accident, injury, or medical emergency involving my child, I authorize Little Sprouts Preschool staff to administer basic first aid and to contact me immediately at the phone numbers provided in this application. If I cannot be reached, I authorize staff to contact the emergency contacts listed above and, if necessary, to call 911 and consent to my child receiving emergency medical treatment, transport, and care by licensed emergency personnel or a physician. I understand that I am financially responsible for any emergency medical care, transportation, or treatment provided to my child. I confirm that the allergy, medication, and medical information provided in this application is accurate and complete, and I agree to notify Little Sprouts Preschool in writing of any changes.

I authorize Little Sprouts Preschool to release my child only to me, to the other parent/guardian listed, or to the individuals named in the Authorized Pick-Up section of this application. I understand that photo identification will be required the first time a new person picks up my child, and that staff may decline to release my child to anyone not listed or to anyone who appears impaired. Changes to the authorized pick-up list must be provided in writing (email is acceptable).

Little Sprouts Preschool may take photos or short videos of daily activities. Your specific permissions from the Permissions section of this application govern how they may be used — photos without faces may be shared on our public pages only if you selected "Yes," and photos with faces are shared only within our private family updates if you selected "Yes." You may withdraw or change this consent at any time by emailing us, and we will stop using new media accordingly.

Schedule & attendance: Classes meet on Tuesdays and Thursdays. I understand that my selected schedule (one or two days per week) is reserved for my child for the semester and that tuition is not reduced for missed days, holidays, or family vacations. Tuition & fees: A one-time, non-refundable enrollment fee of $99 per semester reserves my child's spot. Tuition is $45 per class day, billed monthly in advance. The first month's tuition is due together with the enrollment fee at the time of acceptance. My child's spot is confirmed only once payment is received. Health: I agree to keep my child home if they have a fever of 100.4°F or higher, vomiting or diarrhea within the past 24 hours, an undiagnosed rash, or a contagious illness, and to pick my child up promptly if contacted about symptoms during the day. Withdrawal: I agree to provide two weeks' written notice if we withdraw. The enrollment fee and any tuition already paid are non-refundable.
